Thank you for registering for our Combo X-ray and EFDA certification course.  We have received your application and payment! This course  gives you the information needed to perform both X-ray and EFDA procedures. After completing the online portion, a hands on clinical portion is required in our facility. The online quiz is required to be completed no later than the day prior to reporting to the clinical portion of this course.  Download the Employment Verification Form. This form must be printed and filled out by you AND signed by your employer, then email  to info@dai-fl.com or fax to 1-877-546-4431. This form MUST be received prior to your clinical class date.
