Thank you for your interest in the Expanded Functions certification course. We received your application and payment. The online quiz is required for this course and must be completed no later than the day prior to reporting to class. Students must be familiar with the EFDA duties they are to be certified in order to attend the weekend EFDA course. Students are REQUIRED to know dental charting(tooth numbers, surfaces and how to chart existing restorations) before attending this class.

1. Download the Employment Verification Form. This form must be printed and filled out by you and your employer, returned before the day of class by email info@dai-fl.com or fax to 727-546-3500  for  DAI approval.
2.It is necessary to bring a 4 hole slow speed motor, straight nose cone attachment, an acrylic bur, safety glasses, mask and a basic set up (mirror/explorer/cotton pliers/air-water tip), with you to class. These items are not provided but can be rented for $50 prior to the day of class.
