Thank you for registering for our Radiology certification course.  We have received your application and payment! This is a fast paced course that gives an overview with a hands on clinical portion. This course is meant to give you the certification to go back to your office to continue practicing taking x-rays on patients. An online quiz is required for this course to be completed no later than the day prior to reporting to class.  

Getting Started:

1. Download the Employment Verification Form. This form must be printed and filled out by you AND signed by your employer, then email to info@dai-fl.com or fax to 727-546-3500  for DAI  approval. 
2. Please download and review the quiz study guide BEFORE you begin the online quiz.
3. Reminder! You must complete the online quiz no later than the day prior to class. 
4. Once you receive your certificate please download the BOD Application form. You must mail a copy of the certificate with the completed BOD form and a nonrefundable fee of $35.00.
